Members pay for the FIRL quarterly at $165 (billed the 15th of January, April, July, and October), while non-members pay $330 quarterly.

By submitting this form, I authorize the Florida Association of Insurance Agents and SunTrust to initiate entries to my checking or savings account or credit card as noted above. This authority begins with the next billing for the FIRL, and will remain in effect until I notify you in writing to cancel it in such time as to afford FAIA and SunTrust a reasonable opportunity to act on it.
